Initial Consultation
Your initial appointment will consist of a consultation explaining
your diagnosis and treatment options. Occasionally, surgery
can be done the same day as the consultation. However, a complex
medical history or treatment plan may require an evaluation
and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your examination:
- Your signed surgical referral slip from your dentist and any X-rays dated within one year.
- A list of medications you are presently taking: bisphosphonate drugs or bone strengthening medication (Foxamax, Boniva); blood thinners (Warfarin, Coumadin) or other medications.
- If you have medical or dental insurance, bring the necessary completed forms and insurance cards. This will save time and allow us to help you process any claims.
- Bring with you names and telephone numbers of Primary Care Physician, Cardiologist, etc.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the examination visit. A parent or legal guardian must sign the informed consent before surgery can be performed.
A pre-operative evaluation and physical examination is suggested for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink six (6) hours prior to your surgery. You will also need a responsible adult to drive you home. That person must remain in our office during the surgical procedure. Most procedures last about one and one-half hours. We need the patient’s escort to remain in the office to hear post-operative instructions and be available to take the patient home when the patient is ready to leave. If IV sedation was used, the patient needs to have someone available at home with them five (5) – six (6) hours after being discharged.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, pregnancy, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant "blood thinner" therapy, diabetes pills, etc.) At times, pre-medication with antibiotics or stopping certain "blood thinners" may be necessary prior to surgery.
